You agree that you are bringing your student(s) to Vacation Bible School at St. James Presbyterian Church for all or some of the dates of June 15-19.
In the event of an emergency requiring medical treatment, you hereby give your permission to the licensed physician and/or hospital selected by St. James Presbyterian Church to hospitalize and secure proper treatment for, and to order injections, anesthesia, and surgery for your child. You hereby assume all risk to your child participating in VBS. You agree to insure your child against all injury and damage. In addition, you agree to allow your child(ren) to engage in all activities, understanding that it will include spiritual training.
You understand that St. James Presbyterian retains the right for photos taken during the activities for publicity.
